Purpose
You will create a short report based on your community assessment which provides a critical analysis of the major issues affecting your community’s health and quality of life (QoL), with particular relevance to your assigned aggregate clinical group. (AGES 14-21 OF DEVELOPMENTAL DISABILITY STUDENTS) The emphasis should be on writing a quality report that highlights your ability to identify key points and critically analyze it. Think of this as a report which you would present to the parent’s group at your school, the Board of your placement site, which is specific to the needs of the AGES 14-21 OF DEVELOPMENTAL DISABILITY STUDENTS.
Ensure your report covers the following:
• Introduction and Purpose – Briefly outline your purpose in implementing a Community Health Assessment in your particular neighborhood/ placement site
• Data Collection Methods – Briefly list these as (NON-CONTACT METHOD, MINIMAL CONTACT METHOD, INTERACTIVE CONTACT METHODS)
• Key informants consulted – Who and how many? (Use VICE –PRINCIPAL, 2 EDUCATIONAL ASSITANTS. USE 3 INFORMANTS)
• Summary of Key Findings for Target Community. (Disability developmental students) Demonstrate your ability to critically analyze the information from your Community Placement site to identify 3 key social determinants of health (SUCH AS 1, EDUCATION & LITERACY. 2, SOCIAL SUPPORT NERTWORK, 3, HEALTH SERVICES ) that have the greatest impact on your Target Community Summarize the key assets and capacities; barriers; vulnerabilities and issues of wellness or quality of life in the broader target community that led you to identify these 3 SDOHs
• How Findings from the Target Community (developmental disability students) manifest in the Aggregate Community (AGES OF 14-21 of developmental disability students) Note that while the broader target community may have a number of Social determinants of health (SDOH) issues, how they show up in your aggregate may be different. Discuss the relevance of your target community findings to your particular aggregate group.
