Interprofessional Team

Reading: Chapter 19 in Yoder-Wise (2019) discusses the ability of nurses to work effectively in a leadership role as an integral part of a health care team is paramount. Coordinated and collaborative teams of health professionals are essential in acute care settings but also essential in many community settings such as in schools, planning disaster responses, participating in public health education initiatives, and establishing policy. In your readings, you will explore the evolution of team-based care as well as guiding principles for effective team participation. Concepts such as team building, effective delegation, communication skills, and conflict resolution are important aspects of successful participation and leadership within a team environment (Kowalski, 2019, as cited in Yoder-Wise, 2019). Nurses ability to master these concepts and assume leadership roles within a team is an essential component of developing highly functioning health care teams in order to improve patient outcomes and the overall health outcomes of any population.

In this module, we will analyze the evolution of team-based care, the role of effective teams and team participation, and how team-based care is currently being delivered in various healthcare settings to improve health outcomes. We will also examine the importance of the nursing perspective within a healthcare team.

ASSIGNMENT: your assigned readings discuss the principles of team based delivery of health care and the qualities of a team player. In a 1000-1400 word paper drawing from the literature, discuss the following:

  1. Assess the team qualities you have mastered thus far in your career and those you feel need further development.
  2. Evaluate the health care team of which you are a member. Discuss the health professionals that make up the team and their roles, how well the team functions, and what are the important contributions nursing brings to the team.
  3. Based on the readings, in what ways do you think team-based delivery of care can be utilized more effectively in your work setting to improve outcomes.

My nursing disciplinary team consist of the patient, myself (nurse), the doctor, a nutritionist, social worker, and physical therapist. The setting would be within a nursing home. For example patient is a gtube depdent patient was in a case accident, not resulting in any major injuries.Patient and receives food and medication through gtube only. The doctor orders the medication, however patient continues to lose weight. nutritionist is consulted. after changing the feeding and calories the patient is receiving, the patient begins to gain weight appropriately. To keep muscle strength and prevent weakness, patient continues physicals therapy. patient has reached baseline again is social work is preparing for discharge and helping patient set upm services and resources as needed outside of the nursing home.
